Job Responsibilities

  1. Deliver comprehensive and personalized nursing care to patients in their homes
  2. Assess, plan, and implement effective care plans based on patient needs
  3. Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care
  4. Monitor and evaluate patient progress, adjusting care plans as necessary
  5. Provide education and support to patients and their families
  6. Maintain accurate and timely documentation of patient interactions and care provided
  7. Follow all relevant policies, procedures, and regulations

Qualifications

  • Valid RN license in the state of Michigan
  • Previous experience in home care or a related setting preferred
  • Strong clinical skills and knowledge
  • Excellent communication and interpersonal abilities
  • Ability to work independently and as part of a team
  • Compassionate and empathetic nature
  • Flexibility to adapt to changing patient needs
