About the Role

As a Clinical Pharmacist specializing in Anticoagulation, you will be responsible for:

  • Evaluating patients' medication regimens to optimize anticoagulation therapy
  • Collaborating with healthcare teams to develop comprehensive treatment plans
  • Monitoring and adjusting medication dosages based on patients' laboratory test results
  • Providing patient education on anticoagulation therapy and potential side effects

Requirements

To be considered for the Clinical Pharmacist – Anticoagulation position, candidates must possess the following qualifications:

  • A Doctor of Pharmacy (PharmD) degree from an accredited institution
  • A valid state pharmacy license in the state of Michigan
  • Board Certification in Pharmacotherapy (BCPS) or equivalent
  • Demonstrated experience and expertise in anticoagulation therapy
  • Strong analytical and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team
