If you are a highly skilled and compassionate Licensed Practical Nurse (LPN), we have an exciting full-time Homecare LPN job opportunity available. As a Homecare LPN at Bay Regional Medical Center, you will play a crucial role in providing exceptional patient care in the comfort of their own homes.

Job Responsibilities

  • Conduct in-home patient assessments, monitoring vital signs and overall patient well-being.
  • Administer medication, IV therapy, and wound care as prescribed by the healthcare team.
  • Collaborate with physicians, nurses, and other healthcare professionals to create personalized care plans.
  • Provide emotional support and education to patients and their families to ensure a smooth transition from hospital to home care.
  • Maintain accurate and up-to-date patient records, documenting all care provided.
  • Adhere to infection control protocols and maintain a safe environment for patients.

Required Qualifications

  • Valid LPN licensure in the state of Ohio.
  • Prior experience in home healthcare or a similar setting.
  • Strong knowledge of medical terminology and nursing principles.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Flexibility to adapt to changing patient needs and schedules.
