As a Population Health Nurse in our ACO, you will play a vital role in coordinating care, implementing evidence-based programs, and preventing disease.

Your Role and Responsibilities

As a Population Health Nurse within our Accountable Care Organization, you will have the opportunity to utilize your exceptional skills and expertise to:

  • Assess and identify high-risk patient populations
  • Develop comprehensive care plans tailored to individual patient needs
  • Collaborate with multidisciplinary teams to coordinate and manage patient care
  • Implement evidence-based interventions and programs
  • Educate patients and their families about disease prevention and self-management techniques
  • Monitor health outcomes and evaluate the effectiveness of interventions
  • Participate in quality improvement initiatives to enhance population health outcomes
  • Stay updated on the latest research, best practices, and regulations in population health nursing

Qualifications and Requirements

To succeed in this role, you should possess the following qualifications:

  • Registered Nurse (RN) licensure
  • Bachelor of Science in Nursing (BSN) degree, preferred
  • Experience in community health, care coordination, or population health
  • Strong knowledge of evidence-based practice guidelines
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a multidisciplinary team environment
  • Passion for improving community health outcomes
