Your Role as a Social Worker

As a Social Worker at Bay Regional Medical Center, your primary responsibility will be to provide support to patients and their families to ensure their emotional and social well-being during their healthcare journey. You will work collaboratively with the interdisciplinary healthcare team to develop and implement personalized care plans that address the unique needs of each patient. Your role will involve:

  • Conducting assessments to identify the psychosocial needs of patients and families
  • Providing counseling and emotional support
  • Assisting with discharge planning and arranging appropriate follow-up care
  • Advocating for patients' rights and needs
  • Collaborating with community resources to facilitate access to essential services
  • Participating in multidisciplinary meetings and case conferences

Qualifications and Requirements

Bay Regional Medical Center is seeking a qualified candidate for the role of Social Worker. To be considered for this position, you should meet the following requirements:

  • Master's degree in Social Work from an accredited institution
  • Licensure as a Clinical Social Worker in the state of Iowa
  • Prior experience in a healthcare setting preferred
  • Strong interpersonal and communication skills
  • Ability to work effectively as part of a team
