Responsibilities and Duties

As a Population Health Outreach Specialist (LPN/CMA), your primary responsibilities will include:

  • Engaging with patients and assisting in care coordination
  • Conducting outreach efforts to identify and address population health needs
  • Collaborating with healthcare providers to develop and implement care plans
  • Educating individuals and communities on wellness, prevention, and self-management

Required Qualifications

To qualify for the Population Health Outreach Specialist (LPN/CMA) position, you must meet the following criteria:
