Requirements and Responsibilities
Education and Experience
- Valid Registered Nurse (RN) license
- Minimum of 3 years of clinical experience in hospice or related field
- Prior leadership experience preferred
Responsibilities
- Lead and coordinate the delivery of hospice care services in nursing homes
- Assess patients' needs and develop individualized care plans
- Supervise and mentor nursing staff
- Ensure compliance with regulatory standards and policies
- Collaborate with the interdisciplinary team to develop and implement patient care programs
