Discharge Planning Assistant Job Description

As a Discharge Planning Assistant at Bay Regional Medical Center, you will play a crucial role in ensuring smooth transitions for patients from hospital to home or other care settings. Your responsibilities will include:

  • Assisting the discharge planning team in coordinating patient care services
