Bay Regional Medical Center is seeking a full-time Hospice RN Case Manager to join our compassionate team in Silver Spring. As a hospice nurse, you will play a vital role in providing end-of-life care for patients and supporting their families during a challenging time.
Responsibilities
- Manage a caseload of hospice patients, ensuring comprehensive care plans are in place
- Perform physical assessments and develop appropriate nursing interventions
- Administer medications and treatments as prescribed
- Provide emotional support and counseling to patients and their families
- Collaborate with interdisciplinary teams to ensure coordinated care
- Educate patients and their families on managing symptoms and promoting comfort
- Document care provided and maintain accurate patient records
- Participate in team meetings and contribute to quality improvement initiatives
- Adhere to ethical, legal, and professional standards
Qualifications
To be considered for the Hospice RN Case Manager position, candidates must meet the following requirements:
- Valid RN license in the state of Maryland
- Prior experience in hospice or palliative care
- Excellent clinical judgment and critical thinking skills
- Strong communication and interpersonal abilities
- Ability to work independently and as part of a team
