Job Description
As a Contingent Case Manager, you will be responsible for providing comprehensive care management services to patients in our Out Patient department. You will work closely with a multidisciplinary team to develop and implement individualized care plans, coordinate healthcare services, and ensure the delivery of high-quality, patient-centered care.
In this role, you will assess patients' needs, identify appropriate resources and support systems, and collaborate with healthcare providers to optimize patient outcomes. You will also provide education and support to patients and their families to promote self-care and active participation in their healthcare management.
Responsibilities
As a Contingent Case Manager, your responsibilities will include:
- Conducting comprehensive assessments of patients' physical, emotional, and psychosocial needs.
- Developing and implementing individualized care plans based on assessment findings and in collaboration with the healthcare team.
- Coordinating healthcare services, including referrals, appointments, and follow-ups.
- Advocating for patients and ensuring their needs are met within the healthcare system.
- Educating patients and their families about their conditions, treatment plans, and available resources.
- Monitoring and evaluating patients' progress, adjusting care plans as necessary.
- Collaborating with community organizations and agencies to facilitate access to resources and support services.
- Documenting all patient interactions, assessments, and care plans in accordance with regulatory requirements.
Qualifications
To be successful in this role, you should possess the following qualifications:
- Registered Nurse (RN) licensure in the state of Michigan.
- Bachelor's degree in nursing or related field (preferred).
- Minimum of 2 years of experience in case management or a related healthcare role.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
- Knowledge of healthcare regulations and reimbursement processes.
- Proficiency in electronic medical record systems.
- Commitment to continuous learning and professional development.
