Join Our Team as an Annual Wellness RN

As an Annual Wellness RN at Bay Regional Medical Center, you will play a crucial role in promoting preventative care and improving the overall well-being of our patients. Your expertise and dedication will be instrumental in conducting comprehensive wellness assessments, providing education and counseling, and coordinating follow-up care.

Responsibilities:

  • Performing annual wellness visits to assess patients' health conditions
  • Evaluating medical history, conducting physical examinations, and assessing risk factors
  • Developing personalized wellness plans and goals for patients
  • Providing health education and counseling on preventive measures
  • Coordinating necessary laboratory tests and screenings
  • Collaborating with interdisciplinary healthcare teams to ensure comprehensive care
  • Documenting patient assessments, interventions, and outcomes accurately
  • Monitoring patient progress and addressing any concerns or complications

Requirements:

  • Valid RN license in the state of Michigan
  • Minimum of 2 years of experience in a clinical or healthcare setting
  • Strong knowledge of preventive care guidelines and wellness assessments
  • Excellent communication and interpersonal skills
  • Ability to work independently and in a team
  • Detail-oriented with good organizational skills
  • Proficiency in electronic health record systems
