Job Description

Are you a passionate and empathetic social worker looking to work in community hospice care? Bay Regional Medical Center is currently seeking a full-time Licensed Master Social Worker (LMSW) to join our dedicated team. As a social worker at Bay Regional, you will have the opportunity to provide comprehensive support to individuals and families facing end-of-life challenges.

Responsibilities

As a full-time Social Worker LMSW in our Community Hospice department, you will play a vital role in improving the quality of life for our patients and their families. Your responsibilities will include:

  • Conducting psychosocial assessments to understand the unique needs and challenges of patients and their families
  • Developing personalized care plans that encompass emotional, social, and spiritual support
  • Providing counseling and therapy to patients and their loved ones
  • Collaborating with the interdisciplinary team to ensure coordinated care
  • Advocating for patients' rights and facilitating access to community resources
  • Educating patients and families about coping strategies and end-of-life planning

Qualifications

To be considered for the position, applicants must meet the following qualifications:

  • Hold a valid License in the State of [Insert State]
