About the Position

As the Full Time Social Worker LMSW - Community Hospice at Bay Regional Medical Center, you will play a vital role in supporting patients and their families during challenging times. This position requires strong interpersonal and communication skills, as well as a deep understanding of social work principles.

Responsibilities

  • Provide comprehensive social work services to patients and families in the Community Hospice program.
  • Conduct thorough assessments to determine the needs and psychosocial support required for patients and families.
  • Implement appropriate interventions and develop individualized care plans.
  • Collaborate with multidisciplinary teams to ensure coordinated and holistic care.
  • Offer counseling, emotional support, and resources to patients and their families.
  • Advocate for patients and ensure their rights and preferences are respected.
  • Educate patients and families about available community resources and support networks.
  • Participate in quality improvement initiatives to enhance the delivery of care.
  • Maintain accurate and up-to-date documentation of all interactions and interventions.

Qualifications

  • Master's degree in Social Work (MSW) from an accredited program.
  • Licensed as a Licensed Master Social Worker (LMSW).
  • Prior experience in hospice or palliative care is preferred.
  • Strong knowledge of medical ethics and relevant legal regulations.
  • Excellent communication and interpersonal skills.
  • Ability to handle sensitive and emotional situations with empathy.
